About The Position

LifeStream, Inc. is seeking full-time and part-time Licensed Practical Nurses (LPNs) to ensure the medical needs of individuals with disabilities are met. In this role, you will have the opportunity to assist individuals with daily living skills, including emotional and social support, in their homes and in the community. Requirements

Current LPN license
Valid driver’s license
Access to a vehicle
Ability to pass employment screening process
Maintenance of an acceptable CORI and fingerprinting
Negative drug screen
Use of personal vehicle for work purposes
Commitment to the philosophy of community inclusion
Licensed Practical nurse with current licensure in Commonwealth of Massachusetts.
Demonstrated history of quality work performance and teamwork in previous positions.

Nice To Haves

Familiarity with the needs of individuals with developmental disabilities and brain injury.

Responsibilities

Assess all individuals' medical regimes to ensure that all individuals are referred for and receive health services, which will maximize health and safety.
Ensures that all consumers are referred for and receive services which assist with achieving set goals and maximize physical and emotional well being.
Provide direct care duties to maintain the highest level of care standards.
Maintenance of medical charts, medication ordering regime and other medical supply inventory.
Assess all consumer complaints and overall well being of each individual on a daily basis.
Review doctor’s orders for accuracy.
Adheres to DPH regulations for medication storage, administration and destruction.
