About The Position

This is a part-time position operating as a member of the Community Services Division team. The role involves facility management, client treatment management, and responding to a 24-hour crisis line. The position requires providing parent training and support, ensuring client files are complete, monitoring and managing individualized client plans, and assessing residents for potential placement or referral. The case worker will answer a 24-hour crisis line, assess callers, and make appropriate referrals. They will also provide a range of counseling services and a supportive environment to shelter residents, observing all intake and termination procedures. Familiarity with community agencies, resources, and various housing programs is essential. The role also includes responding to emergencies involving Transitional & Permanent Supportive Housing Program participants.

Requirements

  • High school diploma or equivalent is required for this position.
  • Must be at least twenty-one (21) years of age.
  • Demonstrated capacity to work with individuals/families in crisis situations.
  • Demonstrated capacity to work with persons from diverse ethnic, racial and social backgrounds.
  • Ability to relate to individuals in a warm, non-judgmental manner.
  • Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
  • Strong verbal and written communications skills.
  • Must be fluent in spoken English so clients and other service providers can understand the case worker when advocacy services, counseling, direction, etc. are provided.
  • Passage of complete background clearance, Tuberculosis (TB) Test, and any other required testing or clearances.
  • A valid driver’s license, reliable means of transportation and proof of liability insurance is required.

Nice To Haves

  • A Bachelor’s degree is preferred with preference in areas of the human services field.
  • All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  • A minimum of one (1) year of experience providing direct service in social work, crisis intervention, or counseling is preferred.

Responsibilities

  • Operates as a member of the Community Services Division team.
  • Facility management including implementation of daily routine.
  • Management of client treatment.
  • Responding to the 24-hour crisis line including the use of crisis intervention, assessment, diversion and referral skills.
  • Provides parent training and support in areas such as counseling to help parents learn appropriate techniques to overcome their difficulties with parenting through role modeling appropriate parent/child interactions.
  • Ensures that all client files are complete and contain all required documents (i.e. income affidavit, releases, client plans, case notes, intake forms, termination forms, public aid/IDHS forms, aftercare plans, CSBG forms, etc.).
  • Monitors and assists in writing and management of individualized client plans designed in consultation with the KPHH Supervisor.
  • Assesses and evaluates with KPHH Supervisor residents for potential placement or referral for Transitional Housing, Permanent Supportive Housing Program or Homeless Prevention Services.
  • Demonstrates ability to make sound judgments, provide appropriate client services within the guidelines provided, and implement daily routine of KPHH residence including chore schedules, room checks, and supervision of meal serving and clean-up.
  • Answers a 24-hour crisis line with awareness of agencies within DuPage County and the general metropolitan area, as possible resources for clients in crisis and callers at risk of homelessness.
  • Assesses clients utilizing the guidelines provided and complete the necessary forms and records, and make appropriate referrals for all callers not given intake appointments at KPHH.
  • Implements and provides a range of counseling services depending on client needs (i.e. individual or family counseling, crisis intervention, budget counseling, personal hygiene, money management, job seeking skills, etc.) and provide a supportive environment to shelter residents.
  • Observes all intake and termination procedures for shelter; prepares all necessary forms; case notes, facilitates individualized client plan and follow-up on these plans throughout their shelter stay and termination.
  • Is familiar with community agencies and resources to serve the needs of the population who are at high risk of becoming homeless and who are without shelter.
  • Is familiar with diversion, Homeless Prevention, Rapid Re-Housing, Permanent Supportive Housing and Transitional Housing Programs.
  • Respond to emergencies involving Transitional & Permanent Supportive Housing Program participants; inform TH/PSH staff, the Assistant Director of Community Services of the emergency and any response made to residents.
  • In the event a TH/PSH staff is unavailable, the Assistant Director of Community Services - North should be notified immediately.
  • The Director of Community Services may be notified in the absence of the Assistant Director.
