About The Position

Trillium Health Resources is seeking a Residential Health Specialist to join their team. This role is crucial in ensuring members receive the necessary support within appropriate living environments. The specialist will identify and connect members with suitable residential and other provider agencies based on their individual needs. The position is part of the Tailored Plan and Managed Care Organization (MCO) serving 46 counties in North Carolina, focusing on individuals with serious mental health needs, substance use disorders, traumatic brain injuries, and intellectual/developmental disabilities. Trillium aims to help individuals and families build strong foundations for healthy, fulfilling lives.

Requirements

  • High School Diploma or GED and a minimum of one (1) year working experience focused on working with individuals with Intellectual Developmental Disabilities (IDD) or Traumatic Brain Injury (TBI), or BH/MH/SU, or Innovations populations; OR Equivalent combination of education/experience.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s catchment area which includes North Carolina counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond.
  • Must be able to travel within catchment as required.

Responsibilities

  • Locate residential and other provider agencies for members enrolled in Tailored Care Management.
  • Work closely with the Primary Care Manager and Regional Care Teams to locate appropriate residential and other supports based on member’s level of care needs.
  • Complete required documentation/paperwork/tasks in a software platform according to required timelines.
  • Assist with integrated care management through multi-disciplinary collaborations and referrals.
