About The Position

Burlington United Methodist Family Services, Inc. d/b/a Star of Hope Family Services is seeking a compassionate, organized, and dedicated Case Manager to join our Residential Treatment team. As a Case Manager, you'll serve as a primary advocate for youth in our residential program by coordinating services, supporting treatment planning, collaborating with families and community partners, and helping ensure every child receives the care and resources they need to succeed. Working alongside therapists, nurses, residential staff, educators, and community agencies, you'll play a vital role in creating positive outcomes for youth through trauma-informed, relationship-based care. Requirements

Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a closely related field.
Valid driver's license with an acceptable driving record.
Ability to successfully complete required background screening.
Ability to respond to and manage crisis situations.

Interpersonal relationship skills.
Ability to communicate orally and in writing.
Ability to work effectively in a fast-paced environment.

Nice To Haves

West Virginia Social Work License (LSWA, LGSW, or higher).
Experience in one or more of the following: Residential treatment, Child welfare, Behavioral health, Case management, Juvenile justice, Foster care, Community mental health.
Experience working with trauma-informed care, multidisciplinary teams, and state child welfare systems is highly valued.
West Virginia Social Work license is preferred.

Responsibilities

Coordinate case management services for youth receiving residential treatment.
Develop and maintain supportive relationships with youth and their families.
Schedule, coordinate, and document Treatment Plan Meetings.
Conduct regular individual meetings with assigned residents.
Coordinate services with therapists, physicians, psychologists, schools, juvenile probation, child welfare agencies, and other community partners.
Complete required assessments, including CAFAS, APS Care Connect, and other clinical documentation.
Maintain timely, accurate case notes, monthly summaries, home visit documentation, court reports, and treatment records.
Communicate regularly with the West Virginia Department of Health and Human Resources (DHHR) and other referral agencies.
Attend multidisciplinary team meetings, court hearings, and MDT meetings as needed.
Assist with discharge planning and securing appropriate placement options for youth.
Monitor documentation for completeness and submit required billing documentation each month.
Respond appropriately to crisis situations and communicate significant incidents to appropriate agencies and team members.
Collaborate with the Clinical Team to review incidents and support ongoing treatment planning.
Help maintain compliance with agency policies, licensing standards, and documentation requirements.
Support Star of Hope's mission by promoting professionalism, teamwork, confidentiality, and respect in every interaction.
Transport youth as needed in accordance with agency policies.
