Requirements

  • 24 months training or 24 months' experience in providing direct care to patients &/or in participating in development &/or carrying out of habilitation/treatment plans for patients who had either developmental disabilities or mental disability
  • 6 months of required training &/or experience should have been as ward charge or team leader with responsibility for ensuring other workers performed their duties & ensuring safety of sanitation of assigned work area.
  • Equivalent of Minimum Class Qualifications for Employment noted above.

Responsibilities

  • Coordinates and oversees implementation of active treatment programming and residential services for individuals residing in assigned areas
  • Participates directly or indirectly in development and implementation of individual active treatment plans in cooperation with professional & para-professional staff by attending meetings, participating in evaluations
  • Provides support related to Electronic Health Record (EHR) processes including assisting staff with EHR documentation, reinforcing documentation expectations
  • Collaborates with interdisciplinary team members to ensure continuity of care and services
  • Approves/disapproves leave requests, makes assignments, oversees & monitors KRONOS timekeeping system
