About The Position

Harvest Home is a Christian non-profit organization dedicated to equipping and empowering unhoused pregnant women for motherhood through housing, support, and holistic programming. The Residential Care Manager plays a crucial role in supporting residents throughout their entire journey, from entering the Residential Program to transitioning into housing and beyond via the Alumnae Program. As case managers, Care Managers collaborate with each resident to create personalized care plans tailored to their unique needs, goals, and experiences. This involves daily meetings with residents, connecting them to essential resources, and ensuring continuity of care across the organization. Care Managers are integral to a continuum of care model designed to minimize relational transitions for residents as they navigate the two-step residential program (Step 1: Stabilization, Step 2: Preparation for Birth and Motherhood) and the Alumnae Program for ongoing stability. The role also includes case management and services for families, including expectant mothers with children up to age 5.

  • Ability to set, model, and uphold appropriate personal and professional boundaries and relationships with staff, residents, volunteers, and community providers.
  • BSW with 2+ years relevant experience preferred; MSW; or other equivalent experience considered.
  • 2+ years case management experience with a similar population in a relevant (residential) setting, preferred.
  • Experience working with diverse, high-risk populations.
  • Strong understanding of and experience with the complexity of poverty and women’s homelessness, mental illness, trauma, substance abuse, health issues, permanent supportive housing model, and evidence-based practices.
  • Preference given to candidates with human services experience in LA County and with knowledge of local agencies and organizations.
  • Mature Christian faith, strong discernment, and model of servant leadership.
  • Insured and reliable personal transportation, valid California driver’s license, and clear driving record.
  • Smart phone that is compatible with work-related apps.
  • Comprehensive employment and criminal background check.

Responsibilities

  • Provide individual care coordination for pregnant, recently postpartum, and graduate mothers (new mothers transitioned into housing) and their children (newborns to age 5 for the Harvest Home Family Program).
  • Meet with residents one-on-one to set individualized goals, develop care plans, assist in overcoming barriers, and connect them to necessary resources such as housing, employment, education, wraparound services, public benefits, and identification.
  • Collaborate with other Care Managers to lead weekly group Care Coordination sessions to discuss residents and alumnae with the Care Continuum team (residential life staff, therapists, Residential Leadership), focusing on resident strengths, needs, and program standing to ensure optimal functioning within the residential program, including upholding household requirements, establishing accountability, and following through on areas for change, modification, or engagement.
  • Facilitate the Critical Time Intervention case management model for individuals within their first year post-graduation from Harvest Home, including conducting home visits.
  • Listen attentively to resident concerns and suggestions, addressing them fairly and professionally, and coordinating with the immediate supervisor to find solutions.
  • Initiate and implement improvement plans, holding residents accountable where necessary and appropriate.
  • Facilitate linkage to mental health, substance abuse, and/or medical services, providing transportation as needed.
  • Utilize databases and systems for timely, organized, detailed, and accurate input and maintenance of services provided and needed.
  • Advocate on behalf of residents and alumnae for support services in decision-making and self-advocacy.
  • Provide and/or coordinate crisis intervention assistance in alignment with the agency's trauma-informed care approach.
  • Coordinate quarterly resource and class sessions for program graduates with Residential Leadership.
  • Provide financial assistance, resources, and support with budgeting, financial growth, and collecting program fees.
  • Promote personal growth, independence, responsibility, and accountability among residents, inspiring them to reach their full potential.
  • Collaborate with the Residential Life Team on the provision of classes, workshops, groups, and events.
  • Develop and maintain close working relationships with local service providers, outreach workers, case managers, and collaborative agencies.
  • Engage with external organizations to stay informed about new, changing, or additional opportunities to ensure case management services and resources are appropriate, relevant, up-to-date, and applied to resident and program needs.
  • Attend community meetings to stay informed about current resources, information, and community awareness within the maternity/transitional home network, housing resources, employment resources, etc.
