About The Position

Huntington At Symphony provides oversight and management of 25 units of subsidized housing for persons living with HIV/AIDS. This unique program also provides 24-hour concierge service and access to JRI Health Medical Case Management.

Requirements

  • High school diploma or GED required; some college preferred.
  • Certified Nurse’s Aide or Home Health certification preferred.
  • Experience in any of the following areas desirable: Special needs residential work, hospice, and home health care, experience working with the elderly, substance abuse or mental health treatment.
  • Excellent oral, interpersonal and written communication skills.
  • Demonstrated awareness of and sensitivity to issues related to the elderly, mental illness, racial/ethnic diversity, and addiction.
  • Energetic and self-motivated reliability and flexibility.
  • Demonstrated knowledge of and sensitivity to issues related to HIV, substance abuse, poverty & homelessness, and other populations required.
  • Background and driving record checks will be performed.

Nice To Haves

Responsibilities

  • Report to the Program Director and Nurse Coordinators.
  • Working as part of a team in a residential program for fragile elderly residents to provide practical support and assistance with activities of daily living as well as emotional and social support to program residents.
  • Assisting program case managers and program coordinator.
  • Make regular contact with residents to ensure safety and well-being and to monitor needs for support.
  • Become and remain fully familiarized with each resident's situation and needs through review of intake documents, daily log, case management notes, staff meetings and interactions with residents.
  • Assisting residents with personal care (e.g. dressing, bathing, lifting, and transferring as well as with practical support needs (cooking, cleaning, laundry shopping).
  • Accurately record in daily logbook all activities performed with each resident, including personal care and practical support provided.
  • Perform other duties as requested by supervisor.
