Under the supervision of the Supervisor of Residential Services, the Resident Service Coordinator is responsible for providing individual case management services and group programming for senior tenants. This role involves monthly in-home assessments of the social and emotional needs of the tenant and assisting formerly unhoused tenants as they transition back to residing in the community. The coordinator develops and implements a written case plan to provide a full range of social services for the tenant and their family, including individual and family counseling/guidance, preparing information about available health, welfare, education, and recreational services, and facilitating tenant engagement. The position also involves screening tenants for eligibility and assisting with applications for benefits, conducting referrals to various service providers, educating tenants on service availability and rights, and ensuring tenants maintain housing subsidy and benefits. Ongoing responsibilities include evaluating tenant functioning, monitoring risk, assessing current services, updating case plans during in-home visits, and reporting on problematic themes to the Supervisor of Residential Services, especially concerning high-risk tenants or those at risk for eviction. Other related tasks and special assignments are also part of the role.
