Resident Service Coordinator- Residential Services
About The Position
Under the supervision of the Supervisor of Residential Services, the Resident Service Coordinator is responsible for providing individual case management services and group programming for senior tenants. This role involves monthly in-home assessments, assisting tenants transitioning back into the community, and developing comprehensive case plans. The coordinator will also provide counseling, connect tenants with available social services, facilitate support networks, and assist with benefit applications. Additionally, the role includes making referrals to various service providers, educating tenants on their rights and available services, ensuring housing subsidy maintenance, and evaluating tenant functioning. Regular reporting to the Supervisor of Residential Services on tenant issues and consultation on high-risk cases are also key responsibilities.
Requirements
- Graduate of an Accredited College or University with a Bachelor’s in Social Work or other Bachelor's level degree or equivalent, preferably in sociology, gerontology, psychology, or human services.
- Minimum one-year relevant experience required.
- Computer proficiency including Microsoft Office, Google Drive, and electronic case record system.
- Excellent interpersonal skills, including the ability to communicate effectively and concisely both in writing and orally.
- Strong organizational and multitasking skills.
- Ability to understand and appreciate the role of race, religion, ethnicity, sexual preference, and individual values as they relate to service clients and their families

Responsibilities
- Provides monthly in-home assessments of the social and emotional needs of the tenant.
- Assists formerly unhoused tenants as they transition back to residing in the community.
- Develops and implements a written case plan to provide a full range of social services for the tenant and their family.
- Provides individual and family counseling/guidance to resolve any issues the tenant is facing.
- Prepares specific information about the health, welfare, education, and recreational services available to serve the tenant including trainings for tenants regarding the obligations of tenancy.
- Assists tenants in developing informal support networks with other tenants, family, and friends, including facilitating opportunities for tenant engagement and participation in group meetings and recreational activities.
- Screens tenants for eligibility and assists tenants with applications for appropriate benefits and entitlements.
- Conducts referrals to JASA and other providers for the direct, legal aid, protective services, vocational placement and volunteer opportunities, primary care, mental health services, etc.
- Educates tenants on service availability, application and benefit procedures, tenant rights, and provides advocacy when appropriate.
- Ensures tenants maintain housing subsidy and benefits required to remain in permanent housing.
- Evaluates tenant functioning, monitors risk, assesses the status of current services, and updates and revises the case plan at ongoing in-home visits.
- Provides regular reports to the Supervisor of Residential Services on common problematic themes/patterns of the tenants.
- Consults with the Supervisor of Residential Services regarding high risk tenants including those tenants who may be at risk for eviction.
- Performs other related tasks, special assignments, and duties as assigned.
