About The Position

This role involves delivering exceptional, patient-centered care for a diverse Medical-Surgical population. The nurse will utilize evidence-based practice, strong clinical judgment, and autonomy to assess, manage, and respond to complex and changing patient conditions. The position requires synthesizing clinical data to develop, implement, and evaluate personalized plans of care, while executing physician and nursing orders with a focus on safety, quality, and optimal outcomes. The nurse will also function as the central point of coordination for patient care, partnering with physicians and the interdisciplinary team to communicate clinical updates, coordinate orders and consults, and ensure safe, seamless care throughout the patient stay. Additionally, the role includes initiating discharge planning at admission in collaboration with the interdisciplinary healthcare team, coordinating transitions of care, and providing patient and family education to ensure a timely transition to the next level of care.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or jobtransfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time ofhire required.
