Requirements

  • Active LPN or RN license in good standing (required)
  • Leadership or supervisory experience is a big plus
  • A calm, solutions-focused approach to challenges
  • Strong communication and organization skills
  • A genuine passion for resident care and team support

Responsibilities

  • Partnering with physicians and the care team to create and carry out personalized care plans
  • Supporting and mentoring LPNs and CNAs—answering questions, offering guidance, and helping them grow
  • Overseeing admissions, discharges, and transfers to keep things running smoothly
  • Keeping a close eye on residents’ needs and jumping in when something changes
  • Making sure documentation and care plans stay accurate and up to date
  • Participating in care conferences and collaborating with the interdisciplinary team
  • Helping investigate and follow up on incidents to ensure resident safety
  • Supporting infection control efforts and staff education
  • Being part of leadership meetings and helping shape quality care initiatives
  • Sharing in on-call responsibilities and supporting the team when needed
