Requirements
- Active LPN or RN license in good standing (required)
- Leadership or supervisory experience is a big plus
- A calm, solutions-focused approach to challenges
- Strong communication and organization skills
- A genuine passion for resident care and team support
Responsibilities
- Partnering with physicians and the care team to create and carry out personalized care plans
- Supporting and mentoring LPNs and CNAs—answering questions, offering guidance, and helping them grow
- Overseeing admissions, discharges, and transfers to keep things running smoothly
- Keeping a close eye on residents’ needs and jumping in when something changes
- Making sure documentation and care plans stay accurate and up to date
- Participating in care conferences and collaborating with the interdisciplinary team
- Helping investigate and follow up on incidents to ensure resident safety
- Supporting infection control efforts and staff education
- Being part of leadership meetings and helping shape quality care initiatives
- Sharing in on-call responsibilities and supporting the team when needed
