About The Position

Distinctive Living is seeking a full-time Resident Care Coordinator for its Mira Vie at Brick community. This role is responsible for all activities and operations within the Assisted Living community, including managing resident care, overseeing staff, communicating with medical professionals and families, maintaining regulatory compliance, and reporting to executive leadership. The position provides direct supervision of caregiving, ensures residents are treated with respect and dignity, and fosters a homelike atmosphere.

Requirements

  • High School Diploma or equivalent required.
  • Must have prior supervisory/management experience.
  • Must possess state certifications: CHHA/CNA & Certified Med-Tech.

Nice To Haves

  • Associate or Bachelor's Degree strongly preferred in a field related to this position.
  • Minimum three years' experience in assisted living or memory care preferred.
  • Experience with Microsoft Word and Excel preferred.

Responsibilities

  • Ensures all residents are evaluated prior to residency and upon change of condition, presenting evaluations for approval.
  • Communicates with residents, family, and staff to meet resident needs.
  • Participates in care conferences with residents and family members to discuss needs and service delivery.
  • Responsible for screening, hiring, training, and supervising all Care Partner staff and activity staff, including managing performance and ensuring compliance with HR guidelines.
  • Assures that all staff maintain appropriate annual training.
  • Verifies associate hours on time cards and weekly hours reports.
  • Completes care staff schedules to ensure proper coverage and budget adherence.
  • Assigns weekly blood pressures and weights.
  • Monitors staff in medication administration, including audits of medication sheets.
  • Assists in medication management, including administration, ordering, and record maintenance.
  • Ensures proper medication storage and organization.
  • Observes residents' health statuses, takes action on concerns, and reports changes to Director, Health and Wellness.
  • Reviews capabilities of residents who self-administer medications.
  • Conducts pre-move-in screening of prospective residents.
  • Ensures all required documentation, including service plans and nursing assessments, is completed.
  • Performs ongoing assessments of resident physical and psycho-social needs and coordinates with the nurse.
  • Communicates with families regarding residents' personal needs and achievements.
  • Reminds families of scheduled physician, dental, or eye appointments.
  • Assists in organizing and developing service plan reviews.
  • Observes residents, records significant reactions, and notifies appropriate personnel of resident's condition and reactions.
  • Responds promptly to all emergency calls from residents.
  • Performs daily, weekly, and monthly safety and infection control checks.
  • Coordinates monthly staff schedules and arranges replacement staffing.
  • Assures in-house ancillary medical services are scheduled and followed through.
  • Participates in the maintenance of resident records.
  • Shares on-call duties as required.
  • Participates in community functions.
  • Communicates with community professionals.
  • Provides guidance for staff in the provision of the Activity Program.
  • Responsible for inventory control and budget adherence.
  • Responsible for oversight and compliance with the community's CQI Program.
  • Audits medication administration, documentation, resident weights, and reassessments.
  • Responsible for all local, state, and federal rules and regulations as they apply to services delivered.
  • Ensures they and any employees reporting to them work in a safe manner and that unsafe actions are corrected.
  • Assists with investigation, return to work, and management of injured workers.
  • Fosters teamwork, shows initiative, assists others, and has a positive impact.
  • Has working knowledge of the organization.
  • Perform other duties as assigned or needed.
