About The Position

The primary responsibility of the Resident Care Coordinator is to oversee the health and status of all residents, coordinate care with families, outside agencies, and primary care physicians. This role involves assisting residents with health-related issues, delegating tasks to caregivers, and instructing them on daily functions. The coordinator must ensure staff adherence to laws and regulations, maintain high standards of care, and supervise staff.

Requirements

• Two years of experience as a Caregiver in Assisted Living/Memory Care.
• One to two years of experience as an RN delegated med tech.
• Knowledge of the Enriched Senior Living mission, staff, and operations, including commitment to 'person-directed' care.
• Proper lifting techniques must be used to avoid injury to staff or residents.

Responsibilities

• Assist in the preparation and final approval of all service plans and assessments for residents.
• Conduct assessments upon admission, quarterly, and upon change of condition, reviewed with the Executive Director.
• Act as a liaison for health-related issues to volunteers, families, and the public.
• Attend and participate in monthly care conferences and staff meetings.
• Train and monitor staff in the handling and dispensing of medications.
• Ensure accuracy and up-to-date MARs in BlueStep.
• Coordinate delegation of caregivers with the RN delegator.
• Perform LOCA assessments with final review by the Executive Director.
• Lift more than 50 pounds to assist residents.
• Participate in weekend management rotation.
• Approve timecards.
• Participate in recruitment.
• Perform other assigned duties.
• Participate in Admissions and Marketing as needed.
• Maintain team focus and a willing attitude with all staff.
