About The Position

The Resident Care Coordinator acts as a liaison between residents, families, and the care team, ensuring a supportive and safe living environment. This role supports the Health and Wellness Director by assisting with scheduling, staff supervision, and maintaining quality assisted living services. Responsibilities include resolving resident concerns, coordinating care with external providers, and assisting with medication administration and direct resident care when needed.

Requirements

  • MUST HAVE CAREGIVING & MED TECH EXPERIENCE
  • High School Diploma or equivalent.
  • Must meet the state requirements for medication administration in assisted living.
  • Current First Aid and CPR license or ability to obtain.
  • At least two (2) years of experience administering medications and providing care in a professional caregiving setting is required.

Nice To Haves

  • An LVN nursing license is a plus.
  • At least one (1) year of relevant supervisory experience, including staff scheduling, is preferred.
  • Willingness to participate in weekend rotation.

Responsibilities

  • Schedule and assign caregiving and medication technician shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
  • Participate in the recruitment and interview process for new care team members.
  • Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
  • Supervise the medication room and orders, working with medication techs and caregivers.
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
  • Coordinate care planning with home health agencies on-site, working within community and company policies.
  • Coordinate admissions with the sales team, including supervising move-ins.
  • Complete family/resident admission interviews.
  • Facilitate effective communication among residents, their families, and healthcare providers to ensure seamless care delivery.
  • Address residents’ special needs, requests, and complaints.
  • Assist with medication administration and provide direct care to residents as needed.
  • Understand the community's care regulations, as well as state-specific regulations.
