About The Position

The primary responsibility of the Resident Care Coordinator is to oversee the health status and condition of all residents. This role involves coordinating care with families, outside agencies, and residents' primary care physicians. The coordinator will assist residents with health-related issues, delegate tasks to caregivers, and instruct them on their daily functions. Ensuring staff adherence to laws and regulations, meeting and exceeding standards of care, and supervising staff are also key aspects of this position.

Requirements

  • Caregiver in Assisted Living/Memory Care with no less than two years of experience.
  • RN delegated med Tech with no less than 1-2 years of experience.
  • Knowledge of the Enriched Senior Living mission, staff, and operations, including commitment to "person-directed" care.

Responsibilities

  • Oversee the status and health condition of all residents.
  • Coordinate care with family, outside agencies, and residents' primary care doctors.
  • Assist residents regarding health-related issues.
  • Delegate tasks to caregivers and instruct them on their daily functions with residents.
  • Ensure staff adherence to laws and regulations.
  • Meet and exceed standards of care guidelines.
  • Supervise staff.
  • Assist in the preparation and final approval of all service plans and assessments for residents.
  • Conduct assessments upon admission, quarterly, and upon change of condition, reviewed with/by the Executive Director.
  • Act as a liaison for health-related issues to volunteers, families, or the public.
  • Attend and participate in monthly care conferences and staff meetings.
  • Train and monitor all staff in the handling and dispersing of medications.
  • Ensure that all MARs are accurate and kept up to date in the eMAR system.
  • Coordinate delegation of caregivers with the RN delegator.
  • Conduct LOC assessments with the final review of the Executive Director.
  • Participate in weekend management rotation.
  • Approve timecards.
  • Participate in recruitment.
  • Perform other duties as assigned.
  • Participate in Admissions and Marketing as needed.
  • Maintain team focus and a willing attitude with all staff.
