About The Position

The Resident Care Coordinator is responsible for resident service plan and Health Services staff schedules, and other tasks as needed for resident well-being.

Requirements

• Develop and maintain resident service plan review and schedule according to service plan policy and state regulation for timely reviews with the resident and/or responsible party.
• Discusses with Health Services staff any changes needing to be reflected in resident service plans such as: changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences and reports such instances to a licensed nurse, and/or the administrator.
• Demonstrates knowledge of infection control and universal precautions.
• Observes any changes in resident physical or mental status and communicates changes appropriately.
• Displays mature behavior and attitude in speech and action; demonstrates consistently pleasant demeanor, tone of voice, supports the mission statement and approach to care used by the community.
• Maintains an acceptable attendance record; notifies supervisor if late or unable to work in accordance with personnel polices; follows the assigned work schedule.
