About The Position

Southbridge Care Homes provides exceptional acquisition, management, and redevelopment services for long-term care homes and retirement communities across Ontario. As a member of the multidisciplinary team, you will be providing compassionate personal care to residents according to each resident's care plan. Provide housekeeping, laundry, cleaning and recreational services to residents as per schedule. Provide meal/dining services to residents according to shift schedule. This is a part-time role with working hours from 1:00pm-9:00pm, including a rotational weekend schedule, though extended hours may be required from time to time. The environment is fast-paced and deadline-driven, requiring high attention to detail and strong time management skills. The role includes regular use of computer systems and collaboration with cross-functional teams.

Requirements

  • A negative vulnerable sector check completed within 6 months of hire date.
  • Completion of a 2 Step TB Skin test within the last 12 months.

Responsibilities

  • Responsible for providing compassionate and individualized personal care to residents as outlined in resident’s individual care plans and as needed.
  • Responsible for medication administration to residents as per Mar sheet.
  • Responsible for the cleaning of residents’ rooms, and facility as indicated on work routines.
  • Responsible for individual laundry services for residents and facility linens.
  • Responsible for assisting with scheduled recreation activities as per activities calendar.
  • Responsible for setting up, serving and cleanup of tables in the residents’ dining room.
  • Respects residents’ rights, privacy and confidentiality.
  • Deals tactfully and courteously with residents, families and visitors.
  • Responsible for monitoring, reporting and responding to any concerns, changes in residents’ behavior and/or health condition i.e. resident fall, illness.
  • In the absence of registered staff, responsible to call ambulance or hospital if resident appears to require medical attention beyond what we can provide.
  • Responsible for implementing effective safety measures and reporting any unsafe conditions in the building promptly.
  • Responsible for participating in facility fire drills and following fire policies and procedures.
  • Participate in Continuous quality Improvement initiatives and completing assigned audits.
  • Responsible for participating in facility education and training opportunities.
  • Carries out all other duties as directed by retirement home manager or designate.
