As the Resident Advocacy Director (RAD) at Carolina Reserve at Laurel Park, you will serve as a key support in resident care, assisting with clinical coordination, care management, family communication, and Navion Cares implementation to ensure continuity and quality of services. You will spearhead enrollment in Medicare care coordination programs, develop and implement personalized care strategies, and ensure cohesive collaboration across interdisciplinary teams and external partners. Additionally, using your unique training and experience, you will provide vitally necessary behavioral health services to Carolina Reserve at Laurel Park residents.
