As the Resident Advocacy Director (RAD) at Carolina Reserve at Laurel Park, you will be a key partner in resident care - leading clinical coordination, care management, behavioral health support, and the Navion Cares implementation to ensure resident wellbeing and health. You will lead enrollment in Medicare care coordination programs, develop personalized care strategies including behavioral health services, and drive collaboration across interdisciplinary teams and external partners.
