About The Position

Our client, a cardiac care alliance MSO, is hiring full-time virtual Care Navigators to support medically complex patients with cardiac conditions, primarily congestive heart failure (CHF). This non-clinical, non-licensed role focuses on telephonic patient outreach, care plan support, CMS-compliant documentation, and coordination across the care team. The Care Navigator works under the supervision of RN Care Managers and escalates clinical concerns appropriately. The role supports efforts to reduce avoidable hospitalizations and help patients manage their health over the long term. The position is 100% remote and US-only, with the Dallas/Fort Worth area preferred. It is a W-2 role at 40 hours per week, Monday through Friday, during flexible business hours. US business-hour availability with CST/PST overlap is required. The stated rate is $21–$24 USD per hour, based on experience. The role does not include clinical assessment or medical diagnosis; medication prescribing or adjustments; interpretation of labs, imaging, or EKGs; clinical triage or emergency response; in-person or home-visit patient contact; or billing or coding beyond required time-based documentation.

  • Monitor for signs of worsening conditions or care gaps and escalate concerns to the supervising RN.
  • Review and act on population health dashboards to address care gaps, including wellness visits, labs, and symptom monitoring.
