About The Position

Chronic Care Staffing is looking for a compassionate and detail-oriented Certified Medical Assistant (CMA) to join our team as a Remote Care Coordinator. In this role, you serve as a vital link between patients managing long-term health conditions and their clinical care teams. You will provide essential support by conducting monthly outreach, helping patients follow their care plans, and monitoring health data remotely. This is an ideal role for a clinician who wants to make a measurable impact on patient outcomes while enjoying the flexibility of a remote work environment. Your primary goal is to ensure patients feel supported, informed, and engaged in their treatment plans. Success in this role is measured by your ability to build trust with patients, accurately document health interactions, and provide clear communication to the broader care team. Daily: You will manage a schedule of outreach calls to patients, conduct health risk assessments, document clinical interactions in our EHR system, and assist with Remote Patient Monitoring (RPM) tasks. You will act as a primary point of contact for patient questions and medication adherence support. Over Time: You will become a subject matter expert in our care coordination workflows, helping to improve patient engagement metrics and ensuring that quality-of-care standards are consistently met. You will grow your ability to manage a patient panel effectively, anticipating clinical needs before they become urgent issues.

Requirements

  • Active, unencumbered CMA or RMA certification
  • Current Basic Life Support (BLS) certification
  • Proven experience in a clinical or care coordination setting
  • Strong proficiency in EHR systems and virtual documentation
  • Excellent verbal and written communication skills with the ability to explain complex health information clearly
  • A dedicated, HIPAA-compliant, and distraction-free home office environment with high-speed internet
  • Ability to work independently and manage a structured daily workflow without direct supervision

Nice To Haves

  • Prior experience specifically in Chronic Care Management (CCM) or Transitional Care Management
  • Familiarity with navigating community resources to support patient social determinants of health
  • Experience working with quality metrics and CMS compliance standards
  • Demonstrated success in high-volume, fast-paced remote healthcare environments
