We're looking for a compassionate and dedicated Relative Caregiver Care Coordinator to join our team. In this role, you'll be a vital resource for families, helping them navigate complex systems, access services, and build a strong support network.

Requirements

• A bachelor's degree in social work or a related field.
• At least one year of experience working with youth and families.
• A clear three-tier background clearance.

Responsibilities

• Coordinate all aspects of a family's care, from attending court hearings and team meetings to conducting home visits and developing comprehensive service plans.
• Connect families with community resources, ensuring they have the support they need to thrive.
• Teach positive parenting and problem-solving skills, empowering caregivers to feel confident and self-sufficient.
• Provide parent training and crisis intervention when needed.
• Develop and maintain professional relationships with various community agencies to ensure your families get the best possible care.
• Participate in workshops and conferences to enhance your own professional growth.
• Be responsible for timely and accurate documentation.
• May need to transport children and families to appointments.
