About The Position

The Rehabilitation Specialist provides direct care and support for people with serious mental illness who have difficulty maintaining housing through a Residential Rehabilitation Program. The role helps create a safe, therapeutic environment that supports clients’ independence and well-being. Working under general supervision, the specialist provides one-on-one support as clients learn or regain skills for community and independent living and socialization. The position is part of a collaborative service team and includes medication support, transportation, and advocacy at clients’ medical appointments. Staff receive thorough training and ongoing mentorship from senior staff members.

Requirements

• High school diploma or GED.
• Willingness to obtain additional certifications and training as required.
• Valid driver’s license with 3 points or less.
• Access to an insured vehicle.
• Availability to work days, nights, weekends, and holidays depending on program needs.

Nice To Haves

• Bachelor’s degree in a related field is preferred.

Responsibilities

• Provide one-on-one residential counseling to help clients learn or regain skills for community and independent living and socialization.
• Support medication compliance and take responsibility for medication safety, dosage, and cleanliness.
• Collaborate with the service team to develop and implement individual service plans.
• Transport clients to medical appointments and attend appointments with them.
• Advocate for clients during appointments and ensure follow-up instructions are documented and/or followed through.
• Provide support that fosters a safe and therapeutic environment and promotes clients’ independence and well-being.
