Requirements

• BS or AS degree in Human Services field of study OR HS diploma with 5 years of human services work experience.
• Must possess knowledge of current issues and concerns of people with mental illness, rehabilitative training methods, and community resources.
• Have a valid driver’s license and a clean driving record.
• Candidates must be able to pass a comprehensive criminal background check through the NYS Office of Mental Health.

• Ability to work independently and self-motivated.

Responsibilities

• Provides Case Management within our AWARDS tool
• Monitor’s the resident’s daily activities to ensure a safe and equitable environment.
• Teaches and coaches residents on the skills, abilities, and confidence required to live more independently.
• Medication administration.
• Assists residents with managing their medical and therapy appointments.
• Maintains cleanliness of the residence.
