About The Position

SSM Health Day Institute is seeking a dedicated and compassionate Rehabilitation Case Manager to join our health care team. The ideal candidate will be responsible for coordinating and managing comprehensive rehabilitation plans for patients recovering from illness, injury or surgery. The role involves working closely with medical professionals, patients, and their families to ensure that each individual receives the appropriate care and support throughout their recovery journey. As a rehabilitation case manager, you will serve as a liaison between patients, healthcare providers, insurance companies, and community resources. Your goal is to facilitate a smooth and effective rehabilitation process that promotes patient independence and quality of life. Key responsibilities include coordinating all therapy services, insurance authorizations and verifications, tracking visits and plan of care progression. The successful candidate will have strong organization and communication skills, a deep understanding of the rehabilitation process, and the ability to work collaboratively in a multidisciplinary environment. Experience in case management, nursing, or therapy is highly desirable. Requirements

  • Professional Healthcare Degree in either Nursing or Social Work
  • Licensed PT, OT, SLP

Nice To Haves

  • Experience in physical medicine and rehabilitation strongly preferred.
  • Experience in case management, nursing, or therapy is highly desirable.
  • One year medical rehab case management experience

Responsibilities

  • Monitors and assesses patient plan of care from pre-admission to post- discharge.
  • Assists patient/family in goal setting to maximize patient’s independence in both home and community.
  • Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.
  • Coordinating all therapy services, insurance authorizations and verifications, tracking visits and plan of care progression.
