About The Position

Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.

Requirements

• Minimum two (2) years of experience in the SNF/ICF setting preferred.
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license in good standing appropriate to the discipline.
• Availability to work one weekend day and possibly float to other units or facilities based on census and staffing needs.
• Orientation to the rehabilitation department, EMR documentation (Net Health Rehab Optima Epic), and facility workflows provided.

Responsibilities

• Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team, as appropriate to the role.
• Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life, as applicable to the discipline.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation.
• Documenting progress, changes in plan of care, and discharges according to facility, state, federal, and payer requirements.
• Supervise therapy assistants, when applicable to role.
• Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations.
