About The Position

Lakeview Health Services is seeking a dedicated and enthusiastic Rehab Coach to support individuals with mental illnesses in achieving their goals and advancing their lives within a residential setting. The program is Horizon in Ithaca, NY, with a shift schedule of Monday - Friday 9:00 am – 5:30 pm. The agency is a premier partner in the Finger Lakes region, providing housing, support, and rehabilitative services to individuals with mental illness and healthcare coordination for those with chronic mental and physical health challenges. Requirements

  • High School Diploma with two (2) years of relevant experience or an associate degree in human services or related field.
  • Valid NYS Driver’s License.
  • May be required to lift (minimum of 35 lbs.), push, pull, or carry heavy objects.

  • Bachelor’s degree (preferred).

Responsibilities

  • Collaborates, coaches, and aids individuals with mental illnesses in a residential setting.
  • Teaches and supports the skills necessary for optimal success living independently.
  • Utilizes recovery-focused, strengths-based, person-centered practices to assist individuals in gaining skills lost as a result of mental illness.
  • Strives to create a healing environment that respects the perspectives and experiences of individuals, families, staff, and communities.
  • Practices safe, respectful communication and respects individuals’ boundaries and differences.
  • Assists residents in developing and maintaining a structured daily schedule according to individualized goals and preferences using trauma-informed practices.
  • Utilizes motivational interviewing (MI) techniques to provide individual support, training, and assistance in working towards achieving personal goals.
  • Encourages and participates in social, recreational, educational, and cultural activities.
  • Provides resident transportation via Agency vehicle as needed.
  • Assists in general program housekeeping, including hands-on assistance to teach skills needed to maintain personal living spaces.
  • Develops person-centered service plans for individuals on caseload.
  • Assists residents in monitoring progress on stated goals and provides regular updates.
  • Documents interactions with residents as it relates to their service plan goals.
