About The Position

This full-time position is responsible for analyzing and evaluating clinical patient data to determine the most appropriate placement. The role involves performing physical assessments, reviewing clinical information, and interviewing patients/families to justify the recommended level of care. The RN/Liaison will process referrals and admissions to Advocate Christ Medical Center's Rehabilitation acute adult and acute pediatric programs, including internal and external transfers. They will also serve as a liaison to patients, families, team members, and external contacts, communicating issues regarding patient insurance benefits and assisting with maximizing patient benefits. Additionally, the role supports the program's quality and visibility, interprets payor source processes for financial eligibility, and informs social workers or discharge planners of potential patient out-of-pocket expenses.

Requirements

  • BSN
  • RN licensure
  • Minimum of 3 years experience in an interdisciplinary rehab setting
  • Certification in your current area of specialty within 2 years or 2,000 hours.
  • Knowledge of quality program standards.
  • Strong clinical assessment skills

  • Organization and planning skills
  • CPR certified
  • Ability to negotiate with patients, physicians, and allied health professionals.
  • Ability to assist in evacuation of patients in the event of a fire or other disaster.
  • Ability to lift up to 35 pounds without assistance.
  • For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
  • Unique patient lifting/movement situations will be assessed on a case-by-case basis.

  • Prior experience with various age groups is preferred.
  • Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly.

Responsibilities

  • Conduct initial clinical evaluation of all referrals utilizing information from medical record.
  • Initiate Preadmission Screen (PAS) and obtain all necessary chart information for evaluation.
  • Notify physiatrist to review referral records or review telephonically with physiatrist if preferred.
  • Complete pre-certification/certification process for patient admission for rehab program.
  • Maintain log of deferred admissions and reason for deferral.
  • Monitor CMS 60/40% rule compliance for adult inpatient rehabilitation program.
  • Process all external referrals and determine most appropriate method of review.
  • Ensure all necessary medical information from referring facilities is obtained and reviewed with physiatrists and medical stability issues are resolved prior to transfer.
  • Works collaboratively with medical director and Manager of Clinical Operations to ensure all aspects of the admission process operate in accordance with policy and procedure.
  • Notifies Clinical Bed Management and Charge Nurse of pending admission. Ensures date, time, impairment code, and accepting physiatrist is entered into Teletracking.
  • Monitors patient bed assignments with charge nurse to accurately coordinate admissions to unit and ensure medical needs and safety issues are addressed.
  • Effectively communicates problems and/or issues regarding patient insurance benefits to physiatrist and assists other team members with maximizing patient benefits in order to provide necessary treatment.
  • Updates team concerning payor plans and requirements.
  • Conducts inservices/provides information to referral sources for Advocate Christ Medical Center's Rehabilitation program and admission process as needed.
  • Provide support for patient/family and/or significant other in adjustment to disability.
  • Promotes patient safety by reporting of issues through established channels and participating as requested in safety initiatives.
  • Responsible for monitoring and reporting public aid utilization days, CURES reports, deferrals and other various queries.
  • Schedules patients/family members to tour inpatient rehab unit prior to admission.
  • Collaborates with physiatrists in order to ensure knowledge of clinical skills as they relate to admissions.
  • Maintains knowledge of current market trends to assist in program planning.
  • Attends management and clinical workshops pertinent to rehab admission i.e. clinical appropriateness, managed care referral patterns as appropriate.
  • Maintains Quality Improvement standards within Rehabilitation Services.
  • Ensures compliance with rehabilitation goals, objectives, policies and procedures as well as applicable standards, i.e., DNV.
  • Demonstrates compliance with professional ethical codes of conduct and the Advocate Ethical Code of Conduct Policy and Procedure.
  • Inputs outcome data for program evaluation.
  • Provide financial data, assist financial counselor with rehabilitation patient issues and maintain open communication with Business Office and Patients’ Accounts regarding reimbursement and admitting.
  • Notifies physiatrist and other treatment team members of the patient’s benefits prior to admission if patient has restrictions or limitations.
  • Strategize with Rehab team and patient/family to maximize the inpatient/outpatient benefits and facilitate process by ensuring an informed decision.
  • Inform Social Worker or Discharge Planner if patient may incur copays or out of pocket expenses as quoted by insurance or as indicated per medicare terminal.
