About The Position

The Registered Practical Nurse (RPN) will work to their full scope of practice in this primary care setting to provide health assessments; use critical thinking and clinical reasoning skills to identify issues; design health plans; perform nursing interventions; share information collected regarding patients with interdisciplinary team members; connect with community partners and provide visits off site where relevant; provide practice management for physicians; provide telephone and in- person triage according to established practice protocols; participate in appropriate health promotion and prevention services; document appropriately in the patients’ electronic medical record. This role also requires the facilitation and coordination of clinical programs and involvement with system changes and improvements to enhance team and work processes, and patient care; participation in interdisciplinary team development activities; teaching and mentoring family medicine residents, nursing and other health professional students as appropriate; participation in developing curriculum and problem-based learning experiences.

Requirements

  • Current Certificate of Registration from the College of Nurses of Ontario as a Registered Practical Nurse.
  • Minimum 2 years’ experience in a primary care setting.
  • Proven experience leading a project and/or team.
  • Demonstrated interpersonal, communication and team work skills.
  • Demonstrated ability to concentrate on details.
  • Demonstrates judgment in carrying out his/her role or function and is responsible and accountable for acting on the interpretation of data.
  • Competence with computers for patient database, appointment schedules, internet searches, email.
  • Demonstrates critical thinking and decision making in assessment and problem identifications.
  • Demonstrates ongoing personal professional education and development.
