About The Position

Coleman Care Centre has an exciting opportunity for a Registered Practical Nurse to join their team. This is a Permanent Part-Time position with a specific shift schedule and bi-weekly hours. The role involves providing care within a long-term care setting, focusing on resident well-being and quality of life. Requirements

• Maintain current registration with the College of Nurses of Ontario.
• Existing Heart Save Level of CPR.
• Demonstrate an ability to prioritize tasks effectively, properly plan for, and allocate the right amount of time and effort to complete tasks.
• Remain calm and manages stress appropriately.
• Deliver safe administration of medication and treatments with a focus on accuracy, quality and meeting our residents’ quality of life requests.
• Follow plans of care (or service plans) in accordance with the standards of the College of Nurses.
• Able to identify and report any changes in the resident’s well-being to the Charge Nurse or Village leadership team.
• Have an understanding of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector, including the Fixing Long-Term Care Act, 2021, and the Occupational Health & Safety Act (OSHA) of Ontario.
• Support and embrace our resident-centered social culture.
• Be proud to deliver the highest quality of personal care to our residents.
• Work as a key member of our Village and Neighbourhood multidisciplinary team to understand residents’ individual needs.
• Advocate for residents to actively participate in decisions about their care.
• Support new resident move-ins.
• Facilitate transfers to hospital, or any changes in resident medical needs.
• Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
• Proficient communication and computer skills in order to update care plans in the current system.
• Understand the physical, mental, and emotional demands of the RPN role.

• MDS credentials.
• Experience working with people living with Dementia.
• Knowledge of the palliative assessments scale.
• Comfortable supporting residents and their families during end-of-life care.
• Knowledge of relevant policies, procedures, legislation, regulations and best practices applicable to the retirement and long-term care sector (e.g. RAI processes and CIHI, supplies, equipment ordering, etc.)
