We are hiring a Registered Practical Nurse to work on our Assertive Community Transition Team (ACTT-SW), South West program. The Registered Practical Nurse (RPN) works with clients 16 years of age and older who are experiencing significant challenges with activities of daily living, and functioning across all domains – health, educational, vocational, emotional, social, recreational, and financial as a result of serious and persistent mental illness. In collaboration with the ACT team, the RPN is responsible for the management and administration of medication within their scope of practice and in consultation with the attending ACTT psychiatrist. The RPN works collaboratively with other team members, sharing shift management responsibilities. The RPN conducts mental and physical health assessments, develops plans, coordinates and provides treatment and rehabilitation, as well as professional support. In collaboration with their team, the RPN conducts mental and physical health assessments and makes referrals to community resources including community physicians. Using an Empowerment/Recovery approach, the RPN provides support, assistance, counselling, referrals, and education to clients, and with their permission, their families. The RPN will also work closely with the clinic psychiatrist to monitor and provide follow-up care for clients who are transitioning out of services.
