About The Position

The Registered Practical Nurse (RPN) role in the Hospital Partnership Program focuses on providing and coordinating home health nursing services to diverse individuals, families, and groups. This role adheres to the College of Nurses of Ontario Practice Standards, VHA policies, program-specific guidelines, and Community Health Nursing Standards. The program provides care to clients for 90–120 days post-hospital discharge, ensuring safe, personalized, and comfortable care in the client's home. Services include comprehensive assessments, wound care, IV therapy, chest tube management, drain care, and diabetes management. Care is coordinated with hospital partners and VHA Home Care Specialists to align care plans with evolving client needs.

Requirements

Registered and in good standing with the College of Nurses of Ontario (CNO).
RPN diploma from a recognized university or college.
Valid First Aid or CPR Certification.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G2 or G License) and a reliable vehicle for traveling are required.

Nice To Haves

Membership of your Professional Association preferred (WeRPN).
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.

Responsibilities

Assess, monitor, and make care planning and care coordination.
Evaluate care plan interventions through on-going reassessment and evaluation results.
Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
Maintain documentation standards according to the College of Nurses of Ontario.
Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.
Participate in team meetings, as required during the stated availability.
