The Registered Practical Nurse (RPN) role is grounded in a commitment to Client and Family Centred Care. The primary focus is to provide and coordinate home health nursing services to diverse individuals, families, and groups, adhering to the College of Nurses of Ontario Practice Standards, VHA policies, program-specific guidelines, and Community Health Nursing Standards. We are seeking an RPN to support our Hospital Partnership Program in the Toronto Central region. This role involves providing care to clients for 90–120 days post-hospital discharge, ensuring safe, personalized, and comfortable care delivery. Responsibilities include comprehensive assessments, wound care, IV therapy, chest tube management, drain care, and diabetes management. Care coordination with hospital partners and VHA specialists is essential for aligning care plans with client needs.

Requirements

Registered and in good standing with the College of Nurses of Ontario (CNO).
RPN diploma from a recognized college.
Valid First Aid or CPR Certification.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G License) and a reliable vehicle for traveling are required.

Nice To Haves

Membership of your Professional Association preferred (WeRPN).
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.

Responsibilities

Assess, monitor, and make care planning and care coordination.
Evaluate care plan interventions through on-going reassessment and evaluation results.
Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
Maintain documentation standards according to the College of Nurses of Ontario.
Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.
Participate in team meetings, as required during the stated availability.
