About The Position

Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, resident-focused, goal-directed nursing care in accordance with provincial standards of practice and Extendicare’s policies and procedures.

Requirements

  • Current registration and good standing as a Registered Practical Nurse (RPN) in the applicable province or state.
  • Current Basic Cardiac Life Support (BCLS) and First Aid certification.
  • Computer proficiency, including experience with Microsoft Office applications.
  • Current Vulnerable Sector Screening (VSS/VPS) completed within the past six months.

Nice To Haves

  • Experience working in a seniors’ care environment and supervising or collaborating with unregulated care providers is preferred.
  • Education and/or experience in gerontology, rehabilitation, community care, complex care, or long-term care is considered an asset.
  • Strong written and verbal communication skills.
  • Demonstrated critical thinking, clinical judgment, care coordination, time management, and customer service skills.
  • Sound clinical knowledge and nursing assessment skills.

Responsibilities

  • Collaborate as a member of the interdisciplinary care team in accordance with Extendicare’s Professional Practice Model and person-centered approach to care.
  • Complete and maintain accurate resident clinical assessments, care plans, service plans, and documentation, ensuring updates and changes are communicated appropriately.
  • Implement, monitor, and evaluate care plans, making adjustments as required to meet residents’ evolving needs.
  • Assist in scheduling resident care conferences and actively participate in care planning discussions, where applicable.
  • Work closely with the DOC/DHW to support resident admissions, discharges, and transfers.
  • Provide direct nursing care, including medication and treatment administration, while ensuring compliance with regulatory requirements and Extendicare standards.
  • Process physician orders, safely manage medications and narcotics, and administer medications in accordance with regulatory standards, scope of practice, and organizational policies.
  • Facilitate resident and family education to support health outcomes and informed decision-making.
  • Develop, coordinate, manage, and evaluate resident care plans in collaboration with residents, substitute decision-makers, physicians, nursing staff, interdisciplinary teams, and external service providers.
  • Coordinate nursing assignments to ensure efficient, timely, and high-quality care delivery.
  • Respond appropriately to significant changes in resident condition and medical emergencies, including the provision of emergency first aid.
  • Provide leadership by teaching, delegating to, and directing unregulated care providers.
  • Address resident and family concerns promptly and escalate issues to the DOC, DHW, Executive Director, or other departments as required.
  • Support the DOC/DHW in implementing, coordinating, and monitoring Infection Prevention and Control (IPAC), Health and Safety initiatives, Clinical Programs, and Quality Improvement/Safety Plans.
  • Uphold and promote Extendicare’s values, ethics, integrity, and Code of Conduct.
  • Perform other duties as assigned.
