About The Position

VHA Home HealthCare (VHA) is seeking Registered Practical Nurses (RPN) to work alongside our Hospital Partnerships Program in the Eastern region. This is a part-time visiting role requiring a valid (G2 or G) license and a reliable vehicle. The RPN role is grounded in a commitment to Client and Family Centred Care, focusing on providing and coordinating home health nursing services according to professional standards and VHA policies. The role involves assessing, monitoring, and planning care, evaluating interventions, ensuring quality of care, and demonstrating professionalism, leadership, and accountability. RPNs are expected to support safe client care through continuous learning and to communicate client activities and concerns to their nursing supervisor and case coordinator, maintaining documentation standards.

Requirements

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • RPN diploma/degree from a recognized university or college.
  • Valid First Aid or CPR Certification.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
  • A valid Ontario driver’s license (G2 or G License) and a reliable vehicle for traveling is a requirement.

Nice To Haves

  • Membership of your Professional Association preferred (WeRPN).
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.

Responsibilities

  • Assess, Monitor, and Make Care Planning/Care coordination
  • Evaluate care plan interventions through on-going reassessment and evaluation results.
  • Ensure the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with client and families.
  • Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Inform nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
  • Maintains documentation standards according to the College of Nurses of Ontario.
