About The Position

The Registered Practical Nurse (RPN) is responsible and accountable to the Registered Nurse for providing professional care to residents. This role involves directing, organizing, and supervising the daily work routines of Personal Support Workers (PSWs) in accordance with the standards and criteria outlined by the Ministry of Long Term Care and the policies and procedures of Albright Manor. The RPN provides professional leadership, ensures resident safety and well-being, and manages resident charting and documentation.

Requirements

  • RPN Certificate and Current Registration with the College of Nurses of Ontario
  • Possess interpersonal, creative problem solving, conflict resolution, and leadership skills
  • Read, write and understand to follow/carry out instructions in English independently
  • Ability to meet the required physical demands of the job as outlined in the physical demands analysis
  • Vulnerable Sector Police Clearance (current within the past 6 months)
  • 2-Step TB Test (chest x-ray is acceptable as an alternative)

Nice To Haves

  • Previous experience in geriatric nursing
  • Completion of a university/college course in geriatrics
  • Knowledgeable of requirements of College of Nurses of Ontario
  • Computer skills in resident administration/charting

Responsibilities

  • Provide professional leadership to staff reporting within the assigned unit, including decision-making, problem-solving, direction, delegation, and assignment of duties.
  • Establish and maintain collegial relationships to promote team cohesiveness and ensure positive outcomes for residents and families.
  • Adhere to, direct, and supervise staff in accordance with established policies, procedures, and standards covering infection control, health and safety, and emergencies.
  • Complete all pertinent resident charting documentation using the electronic records documentation system.
  • Complete all pertinent resident charting documentation related to RAIMDS, as assigned and required.
  • Maintain confidentiality of resident information and enforce its importance with staff.
  • Provide direct care and daily monitoring of residents experiencing acute episodes of biopsychosocial conditions beyond the scope of PSW practice.
  • Consult with the Registered Nurse/Physician in cases of uncertainty regarding resident health status and implement remedial health measures or arrange for transfer to hospital as directed.
  • Report any changes in health status to the RN for residents under active RN care.
  • Document all nursing observations and care on the resident's chart.
  • Ensure the safe and effective administration of medications to residents, including ordering, receiving, storing, preparing medication administration records, and reporting errors.
  • Be responsible for the immediate day-to-day needs of new residents, including reviewing charts, providing direction to PSWs regarding care plans, and participating in care conferences.
  • Coordinate and participate in regular annual multidisciplinary care conferences.
  • Develop, implement, and monitor the effectiveness of resident care plans within the scope of RPN practice, addressing uncertainties with the RN.
  • Provide quality assurance activities and implement continuous quality improvement in consultation with the Registered Nurse.
  • Monitor staff to ensure compliance with policies, procedures, and standards.
  • Monitor meal times to ensure compliance with established meal practices.
  • Assign staff duties and breaks to ensure maximum coverage of resident areas.
  • Conduct regular inspections of the resident/staff environment, equipment, and hazardous materials.
  • Provide ongoing teaching and supervision of PSWs regarding their assigned tasks.
  • Be familiar with each resident’s health care directives, including resuscitation wishes.
  • Maintain competency in and perform CPR at a basic cardiac life support level.
  • Monitor all residents for whom restraints have been implemented, including hourly evaluation, collaboration with RN and physician, safe application, and participation in plans to reduce restraint use.
  • Comply with all aspects of the Manor’s Health and Safety programs.
  • Address family concerns in a sensitive and timely manner and report unusual concerns or occurrences to the RN.
  • In the absence of the RN, be responsible for the implementation of the Manor’s fire safety plan procedures.
