Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. The RPN works as a member of the interprofessional team, applying a person-centered approach to care. This role involves completing and ensuring resident's clinical care assessments, care/service plans, and documentation are accurate and up-to-date, and communicating changes as required. The RPN implements and evaluates the effectiveness of care/service plans, assists with scheduling and participating in resident's care conferences, and facilitates resident move-ins, move-outs, and transfers. Direct resident care, medication/treatment administration, and documentation are key components, all in compliance with regulatory and Extendicare standards. The role also includes processing orders, safe-keeping drugs and narcotics, and facilitating resident/family education. The RPN plans, coordinates, manages, and evaluates resident care plans in collaboration with residents, families, physicians, nursing, and interdisciplinary teams. They coordinate nursing team assignments for efficient service delivery, respond to significant changes in resident condition and medical emergencies, and provide emergency first aid. Additionally, the RPN teaches, delegates to, and directs unregulated care providers, responds to resident/family concerns, and follows and assists the DOC/DHW in coordinating and monitoring Infection Prevention and Control, Health and Safety, and Clinical Programs.
