Reporting to the Director of Care (DOC), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. This role involves working as a member of an interprofessional team, applying a person-centered approach to care, and ensuring accurate and up-to-date clinical documentation. The RPN will implement and evaluate care plans, assist with resident care conferences, and facilitate resident transitions. Direct resident care, including medication and treatment administration, is a key component, adhering to all regulatory and Extendicare standards. The position also includes processing orders, managing drugs and narcotics, and providing resident/family education. The RPN will coordinate nursing assignments, manage changes in resident condition and medical emergencies, and provide direction to unregulated care providers. Responding to resident/family concerns and collaborating with the DOC/DHW/Executive Director and other departments is essential. The role also involves supporting the coordination and monitoring of Infection Prevention and Control, Health and Safety, and Clinical Programs.
