About The Position

The Registered Nurse provides home health care to patients in Mercer County, Ohio, and surrounding areas. The RN manages patient care from admission through discharge, completes OASIS assessments, develops individualized care plans, and collaborates with an interdisciplinary team to support patient outcomes. The role offers autonomy and flexibility, with the opportunity to build relationships with patients and help them recover safely and comfortably at home.

Requirements

Current Ohio RN license.
Current CPR and/or BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong assessment, communication, organizational, and time management skills.

Nice To Haves

Experience in home health and OASIS assessments is preferred but not required.
Knowledge of Medicare home health regulations and documentation requirements is preferred but not required.

Responsibilities

Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
Conducts comprehensive nursing assessments and develops individualized plans of care.
Coordinates and manages patient care throughout the episode of care.
Provides skilled nursing services, patient education, medication management, and disease management.
Collaborates with physicians, therapists, social workers, and other members of the interdisciplinary team.
Monitors patient progress and adjusts care plans as needed.
Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines.
Participates in quality improvement initiatives and maintains compliance with regulatory standards.
