About The Position

The Registered Nurse (RN) provides home health care to patients in Shelby County, Ohio, and surrounding areas. The RN manages patient care from admission through discharge, performs OASIS assessments, develops individualized care plans, and collaborates with an interdisciplinary team to support positive patient outcomes. The position offers autonomy, flexibility, and the opportunity to build meaningful relationships with patients in their homes. Employment is offered as full-time, part-time, or PRN. Compensation is undisclosed and described as pay per visit, with weekly pay. The employer offers clinical and documentation support, continuing education, and manageable caseloads.

Requirements

Current Ohio RN license.
Current CPR and/or BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong assessment, communication, organizational, and time management skills.

Nice To Haves

Experience in home health and OASIS assessments is preferred but not required.
Knowledge of Medicare home health regulations and documentation requirements is preferred but not required.

Responsibilities

Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
Conducts comprehensive nursing assessments and develops individualized plans of care.
Coordinates and manages patient care throughout the episode of care.
Provides skilled nursing services, patient education, medication management, and disease management.
Collaborates with physicians, therapists, social workers, and other interdisciplinary team members.
Monitors patient progress and adjusts care plans as needed.
Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines.
Participates in quality improvement initiatives and maintains compliance with regulatory standards.
