About The Position

The Registered Nurse (RN) provides care and services to a group of patients, visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the RN provides expertise regarding psychosocial assessments, interventions, and community resources. The RN works in a Case Management, Admissions, or Resource role.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to collaborate in a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work one-on-one with patients while receiving support from a patient care team.
• Ability to work independently and handle phone, email, and written communication, meetings with healthcare professionals, and hospice rules and regulations.
• Time-management skills to manage visits, documentation, meetings, and urgent needs by prioritizing and triaging multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive, reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
