About The Position

Harbors Home Health & Hospice is seeking a compassionate and clinically confident weekend & on-call RN to join their team. This unique position combines daytime patient visits with overnight hospice on-call coverage, providing continuity of care and ensuring patients and families have access to experienced nursing support throughout the weekend. The schedule is Friday-Sunday, with daytime responsibilities including scheduled home health/hospice patient visits and evening/night responsibilities including hospice on-call coverage for urgent patient and family needs. This involves responding to calls, assessing patient needs, providing telephone triage and clinical guidance, and making in-person visits when necessary. The role also requires collaboration with the team, providers, pharmacies, facilities, caregivers, and families, and participation in admissions or other visits as needed.

Requirements

This position requires the RN to remain available and responsive to hospice needs throughout the weekend.
Current WA State RN license
Current CPR/BLS certification
Valid drivers license with acceptable drivers abstract
reliable transportation - or optional company vehicle
ability to work independently and respond appropriately to urgent clinical situations
strong assessment, communication, organization, and critical-thinking skills.

Responsibilities

Provide scheduled home health/hospice patient visits throughout the service area.
