About The Position
Center of Illinois Home Healthcare, Inc. is a Medicare-certified, ACHC-accredited home health agency providing skilled nursing, physical therapy, occupational therapy, speech therapy, and home health aide services throughout Illinois. We are seeking an experienced Per Diem Registered Nurse (RN) to provide skilled home health nursing services to patients in their homes. The ideal candidate has home health experience, strong clinical assessment skills, and knowledge of Medicare home health regulations and OASIS documentation. This position offers a flexible schedule while providing high-quality, individualized patient care.
Requirements
- Active Illinois Registered Nurse (RN) license
- Minimum of one (1) year of home health nursing experience
- Experience with OASIS documentation
- Knowledge of Medicare home health regulations
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment, communication, and time-management skills
Nice To Haves
- Experience using Axxess EMR
- Previous Medicare-certified home health experience
Responsibilities
- Provide comprehensive skilled nursing visits in patients' homes
- Perform patient assessments and develop individualized nursing care plans
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, and Discharge visits
- Accurately complete OASIS assessments and clinical documentation within required timeframes
- Educate patients and caregivers on disease management, medications, treatments, and safety
- Coordinate patient care with physicians and the interdisciplinary care team
- Monitor patient progress and promptly report changes in condition
- Ensure compliance with Medicare, CMS, ACHC, and agency policies and procedures
- Maintain accurate, timely, and complete electronic documentation
- Participate in case conferences and other meetings as needed.
