About The Position

Sentara Leigh Hospital, in Norfolk, VA, is hiring a full time RN Case Manager for our inpatient units house-wide. Generous Sign on Bonus available for qualified candidate! Position Overview Case Manager RN Full-time; 40 hr/week A minimum of 12 months of nursing experience is required. This position could be a good fit IF YOU ENJOY: Providing care to the patient and family over the entire inpatient stay, Developing a relationship, Acting as a clinical leader of the healthcare team, Driving decisions to assure the safest, most effective transition plan for patients and families, Working in a team of nurses and social workers, using each other's expertise, Helping to drive hospital and organizational strategic initiatives in always improving patient care, Utilizing complex critical thinking skills. Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.

Requirements

  • A minimum of 12 months of nursing experience is required.

Responsibilities
