The Coordinated Caregiving (CCG) Registered Nurse plays a critical role in ensuring the safe and effective administration, monitoring, and documentation of medications for individuals served in GT Independence’s North Carolina’s Coordinated Caregiving Program. In addition, the RN is responsible for the oversight, delegation, and training of specialized skilled cares, ensuring that all clinical supports are delivered in accordance with best practices, regulatory standards, and the unique needs of each individual served. This position collaborates with Case Management Entity RNs, Case Managers (CMs), and community partners to support clinical needs and medication safety in accordance with GT Independence policies, DHS regulations, and Coordinated Caregiving guidelines.
