About The Position

This part-time RN role serves the Spring, Texas market. Discovery at Home is part of the Discovery Senior Living family of companies and provides home health services, including Part A, Part B, and Private Duty care. The Registered Nurse provides comprehensive nursing care, assesses patient needs, develops and implements individualized care plans, administers prescribed medications and treatments, monitors patient progress, and coordinates care with an interdisciplinary team. The role also includes patient and family education, emergency response, accurate medical recordkeeping, infection control, and ongoing professional development.

Requirements

• Must be licensed or registered by the applicable state without restrictions.
• Bachelor’s degree in Nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer proficiency and ability to use Microsoft Windows.

Responsibilities

• Provide comprehensive nursing care in accordance with professional nursing standards, policies, and procedures.
• Assess patient needs and develop and implement individualized care plans.
• Administer medications, treatments, and therapies as prescribed by healthcare providers.
• Monitor and document patient progress, including vital signs, symptoms, and changes in medical conditions.
• Collaborate with interdisciplinary healthcare team members to coordinate care and ensure continuity.
• Educate patients and families about treatment plans, medications, and self-care techniques.
• Respond promptly and accurately to emergencies and perform life-saving interventions.
• Maintain accurate, up-to-date medical records, including health history, treatment plans, and progress.
• Follow infection control protocols and help ensure a safe and healthy environment for patients and staff.
• Stay current with nursing practices and attend training and continuing education programs.
