Requirements

  • Active RN license in the state of practice + CPR certification
  • Graduate of an accredited nursing program
  • Minimum of one (1) year experience as a RN; Home health, Med-Surg or Acue Care experience preferred
  • Strong clinical judgment and critical thinking
  • Compassionate communication and patient-first mindset
  • Reliable transportation and ability to travel within the assigned territory

Responsibilities

  • Conduct comprehensive patient assessments and identify care needs
  • Develop and manage individualized plans of care in collaboration with physicians
  • Deliver skilled nursing care, including treatments, medication management, and interventions
  • Monitor patient progress and proactively communicate changes in condition
  • Educate patients and families to support confidence and continuity of care
  • Serve as case manager, coordinating care across the interdisciplinary team
  • Maintain accurate, timely clinical documentation
  • Support and supervise care team members, including home health aides
  • Participate in on-call rotation as needed
