Requirements

Active RN license in the state of practice + CPR certification
Graduate of an accredited nursing program
Minimum of one (1) year experience as a RN; Home health, Med-Surg or Acue Care experience preferred
Strong clinical judgment and critical thinking
Compassionate communication and patient-first mindset
Reliable transportation and ability to travel within the assigned territory

Responsibilities

Conduct comprehensive patient assessments and identify care needs
Develop and manage individualized plans of care in collaboration with physicians
Deliver skilled nursing care, including treatments, medication management, and interventions
Monitor patient progress and proactively communicate changes in condition
Educate patients and families to support confidence and continuity of care
Serve as case manager, coordinating care across the interdisciplinary team
Maintain accurate, timely clinical documentation
Support and supervise care team members, including home health aides
Participate in on-call rotation as needed
