Home Health Registered Nurse (RN), Full Time
About The Position
Providence at Home, a growing division of Providence Life Services, is seeking a Full Time Home Health Registered Nurse (RN) to serve the Oak Lawn, IL area and surrounding communities including Burbank, Tinley Park, Orland Park, Markham, Justice, Oak Forest, and Midlothian, IL. Providence at Home is Joint Commission accredited and has over 30 years of experience providing home-based services. This role involves conducting home health visits within a 25-mile radius of Oak Lawn, IL. The schedule is Monday through Friday during normal business hours, with additional compensation for on-call duties.
Requirements
- Must be a Licensed Registered Nurse (RN) for the State of IL.
- Minimum of 1-2 years’ experience, at least one of which in the area of home care nursing, hospice, ER or Med surg.
- Must be CPR certified.
- Driver's License and good driving record.
- Reliable transportation to travel to patients in the community.
Responsibilities
- Completes an initial assessment of patient and family to determine home care needs.
- Provides a complete physical assessment and history of current and previous illness(s).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
- Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures.
