About The Position

Suncrest Hospice is seeking a Registered Nurse (RN) committed to providing high-quality care to patients and families. The RN provides care and services to a group of patients, visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities. The RN contributes to the interdisciplinary team by providing expertise in psychosocial assessments, interventions, and community resources. The role may be in Case Management, Admissions, or a Resource position.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to work collaboratively within a team environment and one-on-one with patients.
• Ability to work independently on a day-to-day basis.
• Time-management skills to manage visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive across the service area, with reliable transportation, a valid driver's license, and auto insurance.
• Understanding of hospice rules and regulations.

Responsibilities

• Provide care and services to a group of patients.
• Visit patients where they reside, including private homes, skilled nursing facilities, and assisted living facilities.
• Contribute to the interdisciplinary team with expertise in psychosocial assessments, interventions, and community resources.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage patient visits, documentation, and meetings; attend required meetings and respond to urgent needs by prioritizing and triaging multiple concerns.
• Communicate by phone, email, and in writing, and meet with healthcare professionals.
