About The Position

The primary functions of this role are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency. This position assumes responsibility for and participates in the coordination of care. Essential functions include initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs. The nurse will apply specific criteria for admission and re-certification to hospice care, implement and develop the plan of care, and consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs. The role involves prioritizing needs with the IDG/T, providing training to other staff as needed, determining the scope and frequency of services, assessing caregiver ability, initiating preventive and rehabilitative nursing procedures, applying infection control concepts, providing clinical directions to Hospice Aides and LPN/LVNs, preparing clinical and progress notes, ensuring continuity of quality patient care, informing physicians and other personnel of changes in patient needs, monitoring cases for compliance with third-party payors, demonstrating commitment to professional growth, participating in QAPI programs, promoting hospice philosophy and policies, performing on-call responsibilities, and providing effective communication.

Requirements

  • Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
  • Current driver’s license.
  • Two (2) years experience as a registered nurse in a clinical care setting.
  • Must read, write and comprehend English.
  • Reliable transportation and valid auto liability insurance.

Nice To Haves

  • Hospice experience preferred.
  • Specialized skills preferred in palliation and end-of-life.

Responsibilities

  • Administer nursing care for patients of all ages in their place of residence.
  • Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
  • Perform initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs.
  • Apply specific criteria for admission and re-certification to hospice care.
  • Implement/develop/document the plan of care to ensure quality and continuity of care and recommend revisions.
  • Consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Prioritize needs with the members of the IDG/T.
  • Provide training to other staff as needed.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Assess the ability of the care giver to meet the patient’s immediate needs.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Apply concepts of infection control and standard precautions.
  • Provide clinical directions to the Hospice Aide and LPN/LVN.
  • Prepare clinical and progress notes.
  • Ensure continuity of quality patient care delivered with appropriate documentation.
  • Inform physician and other personnel of changes in the patient’s needs and outcomes of intervention.
  • Monitor assigned cases to ensure compliance with requirements of third party payors.
  • Demonstrate commitment and professional growth and competency.
  • Participate in the Hospice’s QAPI program.
  • Promote the Hospice’s philosophy and administrative policies.
  • Perform on-call responsibilities and provide on-call services.
  • Provide effective communication to patients, family, team members, and other health care professionals.
